Profhilo – An Innovative Discovery for Bio-remodeling

As we get older, the levels of hyaluronic acid (HA) that we all naturally have in our bodies decrease. Bitten by an insect? Sue Ibrahim from Elan Medical Skin Clinic in Rayleigh, Essex explains what you should do.

There’s no denying that this summer has been glorious so far, says Sue Ibrahim at Elan Medical Skin Clinic in Rayleigh, Essex. But the sunshine has led to more people than usual being bitten by insects and it seems that the horsefly population has exploded.

According to the BBC, calls to the NHS helpline 111 about insect bites are almost double the rate they normally are at this time of year and senior doctors are reporting incidents of patients being treated in hospital for infected horsefly bites.

One explanation for the rise in the number of horseflies is standing water, such as garden paddling pools, where they and other insects thrive. The advice is to drain paddling pools after use.

What should I do if I’ve been bitten by an insect?

“I find hydrocortisone cream alongside antihistamine tablets works best,” says Sue, a nurse consultant in dermatology with more than 30 years’ experience of treating skin conditions.

“When should you see a doctor? When the bite becomes infected and starts to go red or you develop a high temperature.”

Exposed skin is appealing to insects, so if you are prone to being bitten, we advise you to consider wearing loose-fitting, longer sleeved tops and trousers. Swimming in lakes and rivers is also best avoided.

Unfortunately, there is not much evidence behind plug-in insect repellent devices.

Hair loss can cause embarrassment for men and women – and it can also affect children. Male pattern hair loss is well known but there’s a version that affects women and this can be a very upsetting condition, especially if the bald area cannot be disguised, says Sue Ibrahim from Elan Medical Skin Clinic in Rayleigh Essex.

Patterned hair loss looks different in men and women. In women, there is widely spread thinning of the hair, mainly on the crown. The hairline often remains normal.

What causes female pattern hair loss?

According to the British Association of Dermatologists FPHL is caused by a combination of genetic and hormonal factors. The hairs produced by the affected follicles become progressively smaller in diameter, shorter in length and lighter in colour until eventually the follicles shrink completely and stop producing hair.

FPHL can be associated with conditions in which androgen (a group of hormones) levels are elevated such as polycystic ovarian syndrome (PCOS). Acne, increased facial hair, irregular periods and infertility are all signs of PCOS. It is believed that it can be inherited from either or both parents.

Alopecia

Alopecia is a general term for hair loss. Its full name is alopecia areata. It usually causes small, coin-sized, round patches of baldness on the scalp, although hair in the beard, eyebrows, eyelashes, body and limbs can be affected. In some people larger areas are affected and occasionally it can involve the whole scalp (alopecia totalis) or even the entire body and scalp (alopecia universalis).

Alopecia cannot be cured. Depending on the extent of hair loss there is a good chance that, for 4 out of 5 affected people, complete regrowth will occur within 1 year without treatment. There may, however, be further episodes of hair loss in the future.

What causes alopecia areata?

Hair is lost because it is affected by inflammation. The cause of this inflammation is unknown but it is thought that the immune system, the natural defence that normally protects the body from infections and other diseases, may attack the growing hair. Why this might happen is not fully understood, nor is it known why only localised areas are affected and why the hair usually regrows again.

What is psoriasis?

Psoriasis is a common inflammatory skin disease affecting 2% of the population. It occurs equally in men and women, can appear at any age, and tends to come and go unpredictably. It is not infectious and it does not scar the skin although sometimes it can cause a temporary increase or reduction in skin pigmentation.

Psoriasis can affect the nails and the joints as well as the skin. About half of people with psoriasis have psoriasis affecting the nails. For people with moderate to severe psoriasis about one in three will develop psoriatic arthritis at some time.

Can I treat psoriasis?

Although psoriasis is a long-term condition that cannot be cured there are many effective treatments available to keep it under good control. Sue prescribes the most up-to-date treatment for psoriasis based on the severity of your condition.

The British Association of Dermatologists recommends three approaches, the first being topical treatments including creams, ointments, gels, pastes and lotions.

Phototherapy

Phototherapy is used to clear psoriasis and then keep it away for a period after treatment. Two types of light are used: narrowband ultraviolet B light (nbUVB/TLO1) and ultraviolet A light (PUVA).

Internal (systemic) treatments

Tablet options include acitretin (related to vitamin A), ciclosporin (suppresses the immune system), methotrexate (slows down the rate at which cells divide in psoriasis), and in some centres fumaric acid esters and apremilast. Injectable treatments for psoriasis include etanercept, adalimumab, infliximab, ustekinumab, secukinumab and ixekizumab.

Despite being one of the most widespread skin conditions affecting teenagers AND adult men and women, acne is one of the most poorly understood. There are many myths and misconceptions surrounding acne and its causes. In this month’s blog, our skin expert and nurse consultant in dermatology, Sue Ibrahim, helps separate fact from fiction.

Myth: A poor diet high in fat and dairy causes acne

Scientific research has not found any foods that directly cause acne, so far. However, eating a healthy, balanced diet is essential for your overall health and wellbeing.

Myth: Acne is caused by having dirty skin and poor hygiene

NHS scientists suggest that most of the biological reactions that trigger acne occur beneath the skin, not on the surface, so the cleanliness of your skin has no effect on your acne. And Sue says that washing your face more than twice a day could actually aggravate your skin.

Myth: Squeezing spots gets rid of acne

Although tempting, squeezing spots, blackheads or whiteheads can actually make your acne symptoms worse – and may leave you with scarring. Here are some of the acne treatments that we may recommend at Elan Medical Skin Clinic in Rayleigh Essex:

Myth: Acne is infectious

This is a particularly nasty and hurtful myth that’s totally incorrect. You cannot pass acne to other people or catch it from close contact with someone with acne. The psychological effects of acne are often underestimated by GPs and friends and those affected can suffer from reduced self-esteem and confidence issues.

What causes acne?

The British Association of Dermatologists has a very helpful leaflet about acne, which explains:

The sebaceous (oil-producing) glands of people who get acne are particularly sensitive to normal blood levels of certain hormones, which are present in both men and women. These cause the glands to produce an excess of oil. At the same time, the dead skin cells lining the pores are not shed properly and clog up the follicles. These two effects result in a build up of oil, producing blackheads (where a darkened plug of oil and dead skin is visible) and whiteheads.

The acne bacterium (known as Propionibacterium acnes) lives on everyone’s skin, usually causing no problems, but in those prone to acne, the build-up of oil creates an ideal environment in which these bacteria can multiply. This triggers inflammation and the formation of red or pus-filled spots.

Testosterone

Teenage acne is thought to be triggered by increased levels of testosterone, which occurs during puberty. The hormone plays an important role in stimulating the growth and development of the penis and testicles in boys, and maintaining muscle and bone strength in girls.

The sebaceous glands are particularly sensitive to hormones. It’s thought that increased levels of testosterone cause the glands to produce much more sebum than the skin needs.

How well do you trust reviews placed on websites? You are probably right in assuming that most medical and cosmetic clinics will advertise their more favourable reviews; claiming that they are the ‘Best Clinic in Essex’ and all you have to do is take a look at their glowing patient reviews. There is no doubt that reading client reviews are encouraged, but how reliable are they? Certainly we prefer recommendations from reliable friends but sometimes we need to find a skin clinic quickly; we don’t have the time to ask around. We can take a look at review sites but most of these sites will rank clinics by how much that clinic is prepared to pay to be at the top of the list.

Reliable friends will tell you truth about their experience

Online review sites

When Elan Medical Skin Clinic opened its doors eighteen years ago there was no such thing as online review sites. When What Clinic.com started, we were automatically listed as a medical clinic based in Essex. Gradually we started to receive review of our clinic and all them were published. While the vast majority of those published showed that our patients were satisfied with the service they received, we did receive some reviews that suggested improvements in the way we handled our clients. This constructive criticism helped us to create a more client centred approach to the way worked and we still welcome constructive criticism today.

Facebook reviews

With the rise of social media and Elan Medical Skin Clinic’s own Facebook page, we soon learnt the hard way how damaging a one star review could be. While our happy clients were more than happy to give us 5 stars we were unfortunate to be targeted by Facebook Trolls; these are people that go around leaving one star reviews having never even visited the clinic! These reviews were so frustrating mainly because there is not a lot of things you can do about it. The popular and well know you are, the more likely you are to be targeted by the Facebook Troll.

Trusted reviews

So how do you really get an honest review on how a skin clinic is performing? Well, if the clinic is registered and inspected by a government body as part of the inspection process the inspectors contact random patients and ask for feedback. The Care Quality Commission insist that all patients have the opportunity to provide their feedback to the clinic and if their complaint is a serious one, you can report it directly to them. Treatments You Can Trust are also an organisation that hold a register of inspected cosmetic clinic. They also have a disputes management service, when communications between clinic and client break down. Fortunately we have never had any issues that have required intervention by either The Care Quality Commission or Treatments You Can Trust.
